myhealth365.ca helped people in Mississauga, Milton, South Etobicoke, Oakville and Halton Hills find health care in the right place, at the right time, 365 days a year.

Empower Health designed, built and launched it in 2016 on the iamsick.ca platform. It was a patient navigation partnership with the former Mississauga Halton LHIN and Acclaim Health, the core partners. It was co-designed with local patients, patient-partners and health care organizations in Mississauga.

After 10 years of service (2016–2026), myhealth365.ca is now retired. The same up-to-date information lives on Empower Health’s new sites.
